All documents are available at any Aquanauts dive center and can be completed while onsite, or download them so you can review and sign the necessary areas before you come. Bring signed documents with you to shorten the paperwork process!

• Aquanauts Grenada Brochure
  Review our brochure to review all of the services that we offer at Aquanauts Grenada.
• Divers Liability Release
  Required for all scuba diving activities with Aquanauts.
• Safe Diving Practice
  Required to sign for all scuba diving and scuba course activities.
• Medical Form
  Any dive course offered by Aquanauts requires a completed Medical Form. If you have any questions about medical conditions and your ability to take an Aquanauts diving related course, print this form and review with your Doctor prior to signing up.
• Kids age 8 - 10 may sign up for a BUBBLEMAKER PROGRAM.
  Required to be signed by parents/ guardians and kids. Both Kids and parents need to watch the Risk Flipchart presented by the instructor.
• Youth Responsibility Form
  Required for all scuba diving activities of kids under the age of 15, completed by a parent or legal guardian.
• Grenada Travel Tips
  Review our Grenada Travel Tips for airline, currency, travel, and other valuable information while you are in Grenada.
• Grenada Immigration Form
  Required for all travelers to/from the Island of Grenada.
